HIFEM®

BTL EMSELLA uses HIFEM® (High-Intensity Focused Electromagnetic) technology to deliver supramaximal contractions to the entire pelvic floor at once. A single 28-minute session produces thousands of contractions equivalent to thousands of Kegel exercises — at intensities that voluntary muscle effort simply cannot reach.

The pelvic floor sits deep enough in the body that conscious activation is difficult for most people, and complete activation is nearly impossible. EMSELLA's electromagnetic field penetrates the full pelvic-floor region uniformly, restoring neuromuscular control and re-conditioning weak musculature without the patient needing to learn correct technique.

What a session looks like

Sit down, fully clothed. The chair does the work.

    Before

    Wear loose, comfortable clothing — nothing too tight, since you'll need to be positioned correctly on the chair. Remove jewelry, belts, cards, coins, wallet, watches, and keep phones and other electronics away from the device. No special preparation otherwise.

  2. 02

    During

    You sit at the center of the chair, fully clothed. Feet flat on the floor, knees about shoulder-width, 90° at the hips. The first minute lets you and the operator gauge positioning — the tapping sensation should be over the perineum. Sessions are about 28 minutes. You'll feel tingling, then full muscle contractions throughout the pelvic floor. This is comfortable and tolerable; if anything isn't, the operator adjusts on the spot.

    Six sessions, twice weekly

    BTL recommends six total sessions, scheduled twice a week with at least two days between sessions. Completing the full series is necessary to maximize results. Additional sessions may be recommended depending on the severity of your condition. Results typically continue to improve over the following weeks.

    After

    No recovery time. You return to driving, work, and your full day immediately. Many patients use the 28-minute session to read, take a call, or watch something — the chair quietly handles the rest.

Is EMSELLA only for women?

No. EMSELLA is FDA-cleared for both male and female urinary incontinence. It's used postpartum, after prostate surgery, and for age-related pelvic-floor weakening in any patient. The BTL national patient program includes male testimonial videos — most clinics frame this as a women's-wellness device, which is incomplete.

Do I have to undress?

No. You stay fully clothed the entire session. The chair delivers the HIFEM® field through normal clothing — we just ask you to wear loose, comfortable clothes so you can be positioned correctly.

Does it hurt?

No anesthesia is required and the therapy is generally comfortable. You'll feel tingling that builds into full muscle contractions throughout the pelvic floor. If at any point it's uncomfortable, the operator adjusts the intensity on the spot.

How many sessions will I need?

Six sessions are recommended, scheduled twice a week with at least two days between. Completing the full series is necessary to maximize results. Some patients benefit from additional sessions depending on the severity of their condition — Dr. Libreros makes that call in consultation.

When will I see results?

Many patients notice improvement during the treatment series. Results typically continue to develop over the weeks following the final session. Can I drive home? Take my next meeting?

Yes. No recovery time. You return to driving, work, and your full day immediately after a session. There's no anesthesia and no restrictions afterward.

What if I have a pacemaker, defibrillator, or metal implant?

EMSELLA is contraindicated if you have electronic implants (pacemakers, defibrillators, neurostimulators), drug pumps, or metal implants. Pregnancy is also a contraindication, and treatment cannot occur during active menstruation. We screen for all of this carefully before scheduling any session.

Will my insurance cover it?

Coverage varies by plan and indication. Some insurers cover EMSELLA for documented urinary incontinence diagnoses; many treat it as elective. We help you understand what your specific plan covers during your consultation. Self-pay and financing options are available.

I'm a postpartum mom. Is this safe?

EMSELLA is widely used postpartum for pelvic-floor restoration once you're cleared by your OB. We coordinate with your OB or pelvic-floor physical therapist as part of the evaluation, and screen carefully for any active contraindications before beginning treatment.
