This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Our commitment to your privacy
Neuromag Institute is required by law to maintain the privacy of your protected health information (PHI), to provide you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
We use and disclose PHI for the following core purposes:
- Treatment — providing, coordinating, and managing your care, including sharing information with other providers involved in your treatment.
- Payment — billing and collecting payment from you, your insurance company, or other payers, including workers' compensation carriers and attorneys in medical-legal matters where authorized.
- Health care operations — quality assessment, training, licensing, and the day-to-day administration of the practice.
Disclosures that require your written authorization
Most uses and disclosures beyond treatment, payment, and operations require your written authorization, including most uses of psychotherapy notes, marketing communications, and any sale of your information. You may revoke an authorization at any time, in writing, except to the extent we have already acted on it.
Your rights
You have the right to:
- Inspect and obtain a copy of your medical record
- Request corrections (amendments) to your record
- Request restrictions on certain uses and disclosures
- Request confidential communications (for example, contacting you only at a specific phone number)
- Receive an accounting of certain disclosures we have made
- Receive a paper copy of this notice on request
- Be notified if a breach of your unsecured PHI occurs
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
